呙 is the simplified form of the traditional character 咼, a phonosemantic compound in which 口 ('mouth') provides the meaning and 冎 (depicting a bone joint or skull) the sound. Originally, 咼 denoted a wry or asymmetrical mouth, and later it was borrowed phonetically for the surname Guo. The simplification replaced the internal component 冎 with 内, while keeping the outer 口 frame.
